The undersigned, having acquired the information provided by the processing controller pursuant to art.13 of Law Decree 196/2003, is aware, in particular, that processing will concern the “sensitive” data indicated by art. 4 sub-section1 letter d), and art. 26 of Law Decree.196/2003, i.e., the data “suitable for determining racial and ethnic origin, religious, philosophical and other convictions, political opinions, membership of parties, unions, associations or organisations of a religious, philosophical, political or union nature, as well as data suitable for determining state of health and sex life”.
The undersigned authorises the processing of the data required to perform the operations indicated on the information leaflet and for notifying the data to the parties indicated on the information leaflet;
